In a stunning coincidence, I’ve just discovered three chips have mysteriously appeared on the bottom of my beloved Le Creuset Dutch oven, what a disaster! I’m looking on their web site and apparently there is a lifetime guarantee. Hope it works. Otherwise I hardly dare tell the friend who sold it to me almost unused at a very good price that I have somehow ruined it in just a few weeks. I don’t even know how it happened.
Someone in a student household I lived in once managed to destroy the enamel lining of a Le Creuset casserole dish: it came off in great chips all across the bottom. He didn’t know how that happened, either.
I suspect there may be a failure mode we don’t know about.
I am reading that I should not use it any more as more chips could come off into the food. 
Absolutely you shouldn’t. They are like ground glass, not good for people’s stomachs.

When you think about it, enamel is a sort of glass really. Vitreous, anyway. Says someone who was taught enamelling at one point, as part of the silversmithing. Very high temperatures required.
As I understand it, it’s basically powdered glass melted and frozen into place.
There is a little bit of my mind that insists that the plural of email should be émaux…

They are a joy to cook in and tonight one will be making a burger onion and mushroom casserole as it’s not BBQ weather
Horizontal rain tends to defeat my best BBQ efforts - steamed sausages are not to everyone’s taste!
Late Aged Aunt had one which was crazed to buggery inside. It didn’t survive the journey from Kent to Chesterfield, as 1 chip came out & the rest followed.

I would think it must be about that, though without hoiking it out of a bottom cupboard I can’t be sure…
I’ve realised I must have had it for over 40 years. This makes me feel very old.

It would probably have to be roast leg of a JW for us, since all cold calling is illegal here (phone and in person) with the sole exception of religious organisations.
